The Kootenay Boundary Primary Care Network (PCN) is building a more connected, team-based approach to primary care in our region. We are local clinics, practitioners, partners & patients working together. At the heart of our work is a shared goal: ensuring every resident has access to a trusted care team for comprehensive, culturally safe, and continuous care.
