What is the Kootenay Boundary Primary Care Network?
The Kootenay Boundary Primary Care Network (PCN) is a partnership between local clinics, family doctors, nurse practitioners, the Kootenay Boundary Division of Family Practice, Interior Health, the Ministry of Health, Aboriginal partners, and patients.
Together, we are building a more connected, team-based approach to primary care in the Kootenay Boundary.
Primary care is the care you receive most often — things like check-ups, support for ongoing health concerns, prescriptions and medication support, and follow-up care over time.
What is team-based care?
Depending on where you receive care, your care team may include a family doctor or nurse practitioner, as well as other health professionals such as nurses, social workers, Aboriginal Health Coordinators, dietitians, physiotherapists, occupational therapists, or respiratory therapists.
Care works best when people are at the centre, supported by connected teams.
What does this look like in our region?
The Kootenay Boundary PCN supports primary care across the region by:
integrating team-based providers into clinics and community settings
offering free group education programs
supporting pathways to care for people without a regular primary care provider
working in partnership with Aboriginal and community organizations
providing stewardship for primary care in our region in collaboration with our partner organizations and Kootenay Boundary primary care clinics
Working together to support local care
While PCNs are part of a province-wide initiative, how care is delivered is shaped locally by the needs of people living in the Kootenay Boundary. At the heart of our work is one shared goal: ensuring every resident has timely access to a trusted care team for comprehensive, culturally safe, and continuous care.
